Many studies have outlined a role for food intolerance in the aetiopathogenesis of rheumatoid arthritis through the study of specific diets and elimination programs. This study was a double blind randomized controlled trial of the effect of dietary exclusion to rheumatoid arthritis, based on enzyme linked immunosorbent assays (ELISA) of total serum IgG to food antigens. 18 subjects were recruited to the study, and underwent food hypersensitivity testing by IgG ELISA, baseline assessment, 8-week dietary exclusion and post-intervention assessment. 5 placebo subjects excluded random non-essential foods and 13 active subjects excluded foods isolated by food hypersensitivity testing. 5 subjects completed the placebo diet, and 11 subjects completed the active diet.
The active group showed a significant improvement in self-rated health, pain, non-dominant hand grip strength, Ritchie articular index, number of tender joints, TTW20 and C-reactive protein concentration. No significant improvement was shown in quality of life, duration of morning stiffness, dominant-hand grip strength, number of swollen joints or erythrocyte sedimentation rate. There was no significant improvement in any of the measures in the placebo group. All subjects were hypersensitive to at least one food, and the mode number of hypersensitivities was 3. The most common and most severe hypersensitivity was to milk.
This study concludes that exclusion diets based on the results from ELISA testing for food specific IgG was beneficial in reducing some RA symptoms and signs in RA subjects. However, the prevalence and trends in types of food hypersensitivity were not comparable to data from elimination diets, suggesting that food intolerance and food hypersensitivity are not the same. - presented at
