- BackgroundIn clinical practice, osteopaths, chiropractors and physiotherapists often treat low back pain and dysfunction by applying different non-thrust manipulation techniques, also known as joint mobilisations, that involves oscillatory motion to the spinal vertebrae. Only a few studies have investigated the effect of mobilisation of the lumbar spine on pain pressure threshold, but none have explored the specific effect of harmonic technique.Objectives The aim of this study was to investigate the immediate effects of harmonic technique on pain pressure threshold in the lumbar spine.Methods A single blinded randomised cross-over study was conducted on 18 asymptomatic male subjects. Pain pressure thresholds were measured pre and post intervention (harmonic technique or control) at 1 site at the most sensitive lumbar spinous process.Results There was no statistical significant difference between control and intervention (P=0.928) at any time point. However, a statistical significant increase in PPT was found post treatment, combining both groups, compared to the start of the study (P=0.002).Conclusion A statistical significant difference was observed pre and post intervention in both groups, however a non-significant increase in PPT in the harmonic group was present, which suggests involvement of pain alleviating mechanisms. This study shows a trend towards involvement of therapeutic alliance and placebo effects, influencing the pain pressure threshold positively in both groups.
